Healthcare Reform under the Affordable Care Act has resulted in a new tax form that many medium to large employers are unfamiliar with. If your staff exceeds 50 employees, you will need to provide them with a Form 1095-C . Essentially, it asks for information on offers that have been made regarding health and medical insurance coverage, as well as enrollment in certain health plans. This form needs to be filed no later than February 1st 2016.
